Garage door off track
A door that has jumped its track is unstable and under load. This is the failure most likely to cause an injury, and the one most likely to get worse if you keep trying the opener.

A derailed door is held up by whatever is left. Running the opener again can pull the remaining rollers out and bring the door down. Don't park under it and don't try to lever it back — the tracks bend easily and a bent track turns a repair into a replacement.

Struck by a vehicle
The most common cause by a wide margin. Even a slow bump bends the track or a roller bracket enough to derail the door.

Broken or slack cable
If one cable fails, that side drops, the door goes out of square, and the rollers on the opposite side pull free.

Worn rollers
Rollers that have lost their bearings develop play, and eventually one walks out of the track under load.

Obstruction in the track
Anything hard in the track — a stone, a bolt, a bent section — will lift a roller out as the door passes.
